Please fill out the following form and submit it online to complete the sponsorship process or print the form and fax it to, mail to , or bring the form with you and leave it with the front desk staff. Please note that your sponsorship applies to your animal until that animal is adopted. The one-time cost is $35 per animal (you can also enter a larger amount).
